AB1282 CLINICAL AND RADIOLOGICAL FEATURES IN BRUCELLA SPONDYLODISCITIS: A DESCRIPTIVE STUDY

Background Spondylodiscitis is an infectious inflammation that affects the vertebrae, vertebral discs and adjacent structures. Vertebral involvement during brucellosis considered not only as most frequent localization but also severe one. Objectives The aim of present study was to describe clinical magnetic resonance imaging features brucella spondylodiscitis. Methods We conducted a retrospective including 11 patients who have presented in outpatient clinics physical medicine rehabilitation for spinal with or without neurological disorders from 2016 2021. diagnosis based on symptoms, MRI findings isolation species blood tissue specimens and/or positive Wright agglutination test. Results Eleven were included. mean age 52.5 years [33-74] (6 males 5 female). consumption unpasteurized dairy found 4 patients. main symptom inflammatory back pain it all It associated radiculopathy 8 cases. other symptoms fever 10 patients, sweats cases, weight loss cases hepatomegaly 1 patient. median delay before 5,2 months [1-7]. On examination, we revealed tenderness palpation muscular weakness lower limbs Brucella test ≥1/160 Blood cultures negative Median erythrocyte sedimentation rate (ESR) serum C-reactive protein level 42 mm 15 mg/dl respectively. located lumbar dorsal cervical spine respectively 8, 2 more than vertebrae affected L4-L5 3 followed by T10-T11 Associated sacroiliitis one All images showed hypo intense signal T1 weighted image hyper T2 image. Disc space narrowing documented body osteolysis Epidural collection size up 5cm. Paravertebral peri-vertebral abscesses detected Intradiscal observed Cord compression root nerve noted Biopsy performed bacteriological examination contributory case. received antibiotic treatment combination two three drugs, corticosteroids prescribed four patient underwent surgical intervention. Conclusion Although always specific, may help establish spondylodiscitis even if examinations are negatives [1].
